I acknowledge that my pet(s) have medical conditions and/or are seniors. I am aware that grooming may induce stress, potentially exacerbating existing issues, revealing new ones, or even resulting in a serious medical event or death.

I recognize that my dog may have underlying heath conditions that I may not be aware of such as but not limited to, allergies, parasites, skin infections, or arthritis.
 

I recognize that senior dogs or those with medical conditions may experience mental and behavioral changes, and they are groomed for comfort rather than appearance.
 

I release the Paws 'n Motion from any liability should any problems or medical issues arise.
 

I accept full responsibility for any medical expenses related to my dog, and in the event of an emergency, I authorize the Paws 'n Motion to seek veterinary care at the nearest clinic. I understand that the Paws 'n Motion prioritizes my pet's well-being and will take all necessary measures to ensure its safety.
 

If Paws 'n Motion determines that grooming is too stressful for my pet, the grooming session will be halted immediately, and I will be notified.
